Проведено открытое интервенционное проспективное случай–контроль исследование, включающее 40 пациентов старше 18 лет с декомпенсированным циррозом печени, находящихся на стационарном лечении в ожидании трансплантации печени. Для терапии использовали биомедицинские клеточные продукты на основе мезенхимальных клеток плацентарно-пуповинного комплекса. Методика системной терапии заключалась в продленной инфузии в кубитальную вену суспензии клеток в физиологическом растворе объемом 40 мл системой инфузомат со скоростью 1 мл/мин, доза клеток составила 2,5 (2,0–3,1) млн/кг массы тела пациента. Системное введение клеток не вызвало побочных явлений, не было ассоциировано с осложнениями введения либо ухудшением клинической картины. Лабораторно эффективность терапии подтверждена статистически значимым снижением уровня общего билирубина, отмечен прирост общего белка, а также стабилизация коагулопатии. Из 20 пациентов 8 (40 %) была успешно выполнена трансплантация печени, у 10 пациентов (50 %) наблюдалась стабилизация клинического течения, что позволило продолжить наблюдение на амбулаторном этапе. 2 (10 %) пациента, у которых клеточная терапия была не эффективна, умерли. Показатель однолетней выживаемости в группе клеточной терапии составил 85 % (против 56,5 % в группе сравнения, p = 0,04). Системное введение мезенхимальных клеток плацентарного происхождения является перспективной терапевтической опцией для пациентов с декомпенсированным циррозом печени, позволяющей увеличить сроки ожидания экстренной трансплантации печени и стабилизировать клиническое течение заболевания.</p></abstract><trans-abstract xml:lang="en"><p>Acute-on-chronic liver failure is an acute liver decompensation in cirrhotic patients, which leads to organ failures and high short-term mortality. The treatment is based on the management of complications and, in severe cases, liver transplantation. Liver transplantation is the most effective treatment, but the lack of donor livers and a high cost of transplantation limit its broad application. Human placenta-umbilical cord complex derived mesenchymal stem cells (MSCs) can be considered as an allogeneic source for liver disease. The aim of the study was to assess the safety and potential effectiveness of systemic administration of mesenchymal cells from the human placental-umbilical cord complex for liver failure correction in patients with decompensated cirrhosis of the liver. An open interventional prospective case-control study was conducted, including 40 patients over 18 years of age with decompensated liver cirrhosis who were hospitalized awaiting liver transplantation. Laboratory efficacy of therapy was confirmed by a significant decrease in a total bilirubin level by 39 %, an increase in total protein was noted (by 11 %), as well as stabilization of coagulopathy (decrease in INR by 25 %).
